FIRST VISIT INFORMATION
Before your fist visit please click on the link below to download the NEW PATIENT PACKET. Please complete all forms and bring with you on your initial visit with Dr. Anderson. If you have not seen the doctor for 6 months, OR have a new type or location of pain to discuss with her, please complete the following before your visit.  This will optimize your time with Dr. Anderson and allow her to provide you with the best medical care. 6 MONTH FOLLOW-UP PACKET

Please complete the FOLLOW UP OFFICE VISIT form before each of your subsequent office visits with Dr. Anderson. It will provide it will provide Dr. Anderson with important information about the progress of your treatment. It will also allow the office staff to keep important insurance and contact information updated. Most importantly it will give you an opportunity to review your complaints and your response to the treatments to date, so that our visit time is focused and efficient.

The internet is full of valuable information about your medical conditions. The problem is that it is also full of extreme treatments, alarmist testimonials, exaggerated symptoms and gruesome pictures.

Below is a list of credible, user-friendly websites that I recommend you use to research your health and investigate preventative medicine strategies. Lets spend our limited time together in the office building upon sound information rather than refuting unfounded claims and unsafe recommendations from unscrupulous websites. If there was ever a time to become an educated consumer now is the time.

The following websites are excellent for general medical information and preventative health practices.

1. National Institutes of Health, www.health.nih.gov
This website gives easy-to-read and accurate descriptions of medical conditions, causes, symptoms and treatments. It does this by linking you to trusted websites that provide the specific information you need. I especially like its Healthy Lifestyles section.

2. Mayo Clinic, www.Mayoclinic.com
This website provides very detailed information about specific diseases/conditions. It gives a relatively thorough description of different causes of back pain and its treatments. There are helpful Treatment Decisions sections and a Q&A section called Ask a Specialist. And again, I really like its emphasis on prevention and alternative therapies to treat your pain.

3. Drugs.com, www.drugs.com
Here you can read about your prescriptions and also research the different medications available to treat your medical problems.
This site also provides an easy to use drug interactions checker. It also provides an innovative pill identification wizard that allows you to enter the shape, color and imprint of your mystery pill and view photographs of potential matches to help you identify your medication.

4. Medline Plus, www.medlineplus.gov
As well as offering information on wellness, disease, drugs and other health topics, this site has an extensive medical encyclopedia with cool pictures and diagrams. There is also a medical dictionary that gives spelling and pronunciation tips. Its information is available in more than 40 languages.
Reliable websites recommended for your specific pain disease can be found on this site. For example, there is an extensive listing for fibromyalgia.

5. Cleveland Clinic, www.clevelandclinic.org
This site provides well-organized information on diseases, treatments, procedures, drugs and current research. This site is unique in its use of multimedia formats: check out the very cool interactive tools, video health talks, podcasts and webcasts.

The following websites provide specific information about your pain problems.

1. Spine Universe, www.spineuniverse.com
This site focuses on back and neck pain but gives very in depth discussions about causes, treatments and procedures. There is a FAQ section which answers common questions concerning back and neck pain.

2. St Jude Medical, www.sjm.com
This website offers extensive information for those patients considering either a Spinal Cord Stimulator or a Peripheral Nerve Stimulator. There is an excellent section for patients titled Power Over Your Pain . They even have a Patient Ambassador section to help you connect with other patients with implanted neurostimulators. These patients have volunteered to share their experiences and success with patients considering the procedure. I encourage you to take advantage of this option because decisions are best made with eyes wide open.

3. The National Pain Foundation
All types of pain are discussed on this website. This is also an excellent resource for pain patients providing links for subsidized medications and help finding local pain providers. The mission of the Foundation is to "To improve the quality of life for those living with pain through information, education, and support that connects persons with pain to each other and to those that can help. "

4. The American Academy of Pain Medicine
This is a physician focused website but has a useful Patient Portal that will provide education and help you access a pain physician in your area.

5. American Chronic Pain Association
This website was started by a chronic pain sufferer. I really like the focus of patients helping patients. Who better understands your suffering than other pain patients. There is education on relaxation techniques and other very useful pain management strategies.

6. American Pain Foundation
A non-profit organization, the American Pain Foundation provides many useful links on their website to investigate a wide variety of pain diseases. Their goal is to provide information and support for pain sufferers.
